Australian health workers to close intensive care units in Victoria next week

Members of Australia’s Health Services Union (HSU) will go on strike in Victoria next week in a dispute over stalled wage and career structure negotiations. Over 5000 physiotherapists, speech pathologists and radiation therapists will walk off the job next week, effectively closing the state’s 68 largest health services.

The strike will force the closure of intensive care units and emergency departments across the state.

It is feared the strike could continue into Easter.

National secretary of the HSU, Kathy Jackson said admissions would be crippled, while intensive care patients would have to be evacuated to New South Wales, Tasmania and South Australia as hospitals will not be able to perform tests or administer treatment.

“When an ambulance shows up you can’t admit a patient without an X-ray being available, you can’t intubate them and you can’t operate on them,” she said.

“If something goes wrong in an ICU you need to be able to X-ray, use nuclear medicine or any diagnostic procedure,” said Ms Jackson.

Ms Jackson said the HSU offered arbitration last year, but the state government refused. “They’re not interested in settling disputes, they hope that we are just going to go away.”

“We’re not going away, we’ve gone back and balloted the whole public health workforce in Victoria, those ballots were successful, 97 percent approval rating,” she said.

The HSU is urging the government to commence serious negotiations to resolve the dispute before industrial action commenced.

The government has offered the union a 3.25 per cent pay increase, in line with other public sector workers but the union has demanded more, but stopped short of specifying a figure.

Victorian Premier John Brumby said the claim would be settled according to the government’s wages policy. “The Government is always willing and wanting to sit down and negotiate with the relevant organisations . . . we have a wages policy based around an increase of 3.25 per cent and, above that, productivity offset,” he told parliament.

The union claims it is also arguing against a lack of career structure, which has caused many professionals to leave the health service. Ms Jackson said wages and career structures in Victoria were behind other states.

Victorian Opposition Leader Ted Baillieu said he was not in support of the proposed strike and called on the government to meet with unions. “There could not be a more serious threat to our health system than has been announced today.”

“We now have to do whatever is possible to stop this strike from proceeding,” he said.

The opposition leader will meet with the union at 11:30 AM today.

Victorian Hospitals Industry Association industrial relations services manager Simon Chant said hospitals were looking at the possible impact and warned that patients may have to be evacuated interstate if the strike goes ahead.

Incomplete data may mislead doctors into overprescribing expensive medicines

Medical doctors have not been getting the full picture about newly FDA-approved drugs, concludes a research team from the University of California, San Francisco. This is because not all the studies required for FDA approval get published. New drug studies that do see publication tend to be ones where the medicine appears to perform well while poor and middling results are less likely to appear in medical journals. The result appears to be that doctors who read the available literature may get an inflated impression of new medications and may prescribe expensive new drugs in place of older medicines that perform as well or better. As Jordan Lite of Scientific American wonders, are drug companies cherry-picking the studies they publish to make their drugs look better than they actually are?

The University of California team reviewed trials that had supported new drugs approved from 1998 to 2000 and examined 909 trials of 90 medications. The search was conducted upon PubMed and other search tools that a typical medical doctor or patient could access. They concluded that less than half of the studies had been published five years after drug approval and a publication bias existed.

Erick Turner, who coauthored a similar study earlier this year, expressed concerns to Scientific American that the problem was not merely the raw percentage of studies published, but that a disproportionate share of the research that appeared in journals are examples where new medications appear to perform well:

Ken Johnson, senior vice president of the Pharmaceutical Research and Manufacturers of America (PhRMA), defended the pharmaceutical industry by saying FDA review of new drug applications is more important than publishing the results of medication trials in medical journals. Approved medications come with labels that give patients and doctors enough information, assures Mr. Johnson.

Yet concerns about full and appropriate disclosure have been serious enough that a new law was enacted last year. FDA Amendments Act of 2007 (FDAAA) requires that all trials which support FDA-approved drugs be registered at the National Institutes of Health website. The requirement goes into effect this coming Saturday. Congress enacted the legislation in response to hearings that determined pharmaceutical companies were less likely to publish studies that indicated significant side effects. One shortcoming in the legislation, according to UCSF associate professor Ida Sim, is that the FDA is still not required to specify which trials it weighs when considering applications for drug approvals. Yet she praises the new law as a major improvement. It’s critically important that we know trials exist and that we get the summary results, positive and negative, into the public domain—that’s a huge step and more than any [other] country is doing now.

Wikinews interviews biologist Chris Simon about periodical cicadas

In May, periodical cicadas with 17 years life cycle emerged on the East Coast of the USA after underground development as juveniles since 1996. Researchers and scientists worked to map and study the rare wave, and the locals prepared for the noisy event. First recorded in 1666, the Magicicada septendecim species recently emerged in 1979, 1996, this year, with a next wave due in 2030.

This week, Wikinews interviewed Chris Simon, an ecology and evolutionary biologist at University of Connecticut, about the cicadas.

((Wikinews)) What caused your initial interest in periodical cicadas?

Chris Simon: As an undergraduate student, I was interested in the formation of species so when I went to graduate school I looked for a study organism that was likely to be in the process of forming new species. I chose periodical cicadas because they are broken up into reproductively isolated broods (or year classes). Reproductive isolation leads to speciation so I planned to study biochemical differences among the broods.

((WN)) You study the emergence of the periodical cicadas. What do you study? What observations are you making?

CS: We record exactly where each cicada population emerges (using GPS automated mapping and crowd sourcing). We record the presence or absence of each of the three morphologically distinct species groups of periodical cicadas (Decim group, Cassini group, and Decula group). We collect specimens for DNA analysis. We look for cicadas coming up one and four years early and late. We dig up cicada nymphs and monitor their growth rates.

((WN)) What equipment do you use?

CS: Nets, shovels, automated GPS recorders, cameras, laptop computers, automated DNA sequencers.

((WN)) Do you study the periodical cicadas with anyone else? What is their role?

CS: Yes, there are a large number of people studying periodical cicadas in my lab and in other labs. My lab is made up of Research Scientists, Postdoctoral Researchers, a technician, graduate students, and undergraduates. Research Scientist John Cooley is the leader of the GPS mapping project; he invented the automated GPS recorder; he built our crowd-sourcing website, and he is instrumental in public outreach. Postdoctoral research David Marshall also participates in the mapping project and leads the part of the research related to the mapping of stragglers. John and Dave and Technician Kathy Hill all study periodical cicada mating behavior and conduct mating and hybridization experiments. One of my graduate students Beth Wade has participated in the nymph collections and will soon start genetic work involving genome wide association mapping designed to locate genes related to life cycle. My graduate student Russ Meister is studying the genes of the bacterial endosymbionts of cicadas. My current undergraduate honors student Erin Dwyer is also studying the development of Magicicada nymphs and is helping to design a lab exercise for college students around the eastern US to do the same. CS: We are collaborating with Teiji Sota at the University Kyoto and Jin Yoshimura at Shizuoka University in Japan. They are studying the phylogeography of Magicicada. We are collaborating with John McCutcheon of the University of Montana who is studying the endosymbiont genomes.
CS: We are also collaborating with ecologists Rick Karban and Louie Yang, both professors at UC Davis who have an interest in cicada population dynamics and nutrient cycling in the ecosystem.

((WN)) You studied the periodical cicadas in 1979 and 1996 too. What changes with time?

CS: I have studied periodical cicadas since I was a student back in 1974. What changes with time is increased human development constantly shrinking the patch size of cicada populations.

((WN)) What are your thoughts on the long life span of the periodical cicadas? Why could it be so? What advantages and what disadvantages does it have?

CS: Most or all cicadas have long life cycles compared to your typical annual insect. Examples have been found of two-year to 9-year cycles in different species. Periodical cicadas evolved an even-longer life cycle and I think that part of this relates to the evolution of their synchronized life cycles and peculiar safety-in-numbers strategy for survival. To become synchronized, periodical cicadas had to evolve an exact length life cycle and all adults would have to appear in the same year. Because the nymphs grow at different rates underground, a longer life cycle and a way of counting years must have evolved so that the individuals that get to the last nymphal (underground juvenile) stage first would wait long enough for all other individuals in the population to become ready to emerge.

((WN)) News reports mention this is ‘Brood II’ of the periodical cicadas. What are the distinctive features of this specific species and what is its full scientific name?

CS: The same species exist in multiple broods. No species is restricted to Brood II. The three species present in Brood II are: Magicicada septendecim, M. cassini, and M. septendecula. These same three species are found in every 17-year brood (except the farthest north which only has M. septendecim).

((WN)) At what depth do the cicadas juveniles live underground?

CS: Most live within the top foot of soil but some have been found deeper. We do not know if they go deeper in winter. We need to do much more digging to understand the nymphs.

((WN)) How do people prepare for the cicada emergence?

CS: Of course various people prepare in different ways. Ideally, everyone prepares by studying information available on the web (especially on our websites Magicicada Central and Magicicada.org).

((WN)) Do cicadas affect transport in the local area?

CS: No, not really. Occasionally individuals can be seeing flying across highways and sometimes they smash into cars.

((WN)) Do cicadas usually stay outside or do they also invade houses too?

CS: They stay outside. One might accidentally fly in through an open window but that would be rare.

((WN)) What do the cicadas eat?

CS: Cicadas suck xylem fluid (the watery fluid coming up from the roots of plants) in deciduous forest trees and herbs. Essential amino acids in the cicada diet are supplied by their bacterial endosymbionts. There are two species of endosymbionts. One makes 8 essential amino acids and one makes two essential amino acids.

((WN)) Do cicadas damage crops or city vegetation? What damage?

CS: Cicadas do not chew leave so they do not damage crops like other insects. They can inflict some damage by their egg laying. Cicadas lay eggs in pencil-sized tree branches. If there are not enough branches available, too many female cicadas may lay eggs in a single branch weakening it and making it susceptible to breakage by wind. This can sometimes cause damage in fruit orchards. If the branches break, the eggs die so this behavior is selected against by natural selection.

U.S. Senate approves revised bailout package after controversial additions

The U.S. Senate passed a revised bailout bill designed to help the struggling U.S. financial economy, which has measures nearly identical to the bill rejected by the U.S. House of Representatives on Monday.

“Senate Democrats and Republicans believe it is essential that we work quickly on this important legislation to restore confidence to our financial system and strengthen the economy,” said Senate Majority Leader Harry Reid.

The new revisions include raising the FDIC insurance cap to $250,000, a move designed to please progressives. However, the $110 billion in tax breaks, earmarks and what has been called pork barrel spending is not offset by any increases in revenues and has added opposition to the bill from some Representatives in the House.

Earmarks added into the bailout bill included $192 million in tax rebates for the Virgin Islands rum industry, $148 million in tax cuts for the wool industry, $100 million tax cuts to the auto racing industry, and $48 million in Hollywood tax incentives.

Vice President of Taxpayers for Common Sense, Steve Ellis, offered his explanation for the pork and earmarks added in. “People who support some of these provisions will forget about the $700 billion and concerns they may have on that, and say, ‘If you give me a few million in tax breaks for my constituents, I’ll go along'”.

The tactic seems to have worked, however, managing to flip enough votes to pass the bill.

“The inclusion of parity, tax extenders and the FDIC increases has caused me to reconsider my position,” said Representative Jim Ramstad (R Minnesota), who voted against the previous bill on Monday. “All three additions have greatly improved the bill.”

But Representative Marcy Kaptur (D Ohio) was not changing her no vote. “I will not support this legislation because it’s the wrong medicine,” she said.

The Senate took H.R.1424, a bill originating in the House concerning “equity in the provision of mental health and substance-related disorder benefits under group health plans, to prohibit discrimination on the basis of genetic information with respect to health insurance and employment,” and extended it with the bailout provisions.

H.R.1424 was introduced on March 9, 2007, by Rep. Patrick Kennedy (RI-1) and had the support of First Lady Rosalind Carter. It is noted on the Congressional Website that “On 10/1/2008, the Senate passed H.R.1424 as the vehicle for the economic rescue legislation. In the EAS version of the bill (Engrossed Amendment as Agreed to by the Senate), Division A (pp.1-110) is referred to as the Emergency Economic Stabilization Act of 2008; Division B (pp. 110-255) is referred to as the Energy Improvement and Extension Act of 2008; and Division C (pp. 255-441) is referred to as the Tax Extenders and Alternative Minimum Tax Relief Act of 2008.” It was not treated as an appropriations bill in the House.

There were two votes in the Senate. The first was to amend H.R.1424, which required 3/5 to be accepted, which it was. The second was a vote on the bill. Passage of the Bill required only a 1/2 majority. It was passed with 74 yeas and 25 nays. Senator Kennedy did not vote.

Viktor Schreckengost dies at 101

Viktor Schreckengost, the father of industrial design and creator of the Jazz Bowl, an iconic piece of Jazz Age art designed for Eleanor Roosevelt during his association with Cowan Pottery died yesterday. He was 101.

Schreckengost was born on June 26, 1906 in Sebring, Ohio, United States.

Schreckengost’s peers included the far more famous designers Raymond Loewy and Norman Bel Geddes.

In 2000, the Cleveland Museum of Art curated the first ever retrospective of Schreckengost’s work. Stunning in scope, the exhibition included sculpture, pottery, dinnerware, drawings, and paintings.

Typhoon Phanfone strikes Philippines

Typhoon Phanfone made landfall on the central Philippines from Tuesday night through to Wednesday, killing at least 20 according to Philippine officials on Thursday, and leaving reportedly at least 15,000 stranded at ports.

Phanfone impacted upon popular tourist attractions, such as Boracay and Coron, as well as the Kalibo International Airport. It also blocked roads and forced over 58,000 civilians to evacuate.

Phanfone damaged similar regions to Typhoon Haiyan, which killed over 6,000 people in 2013. Cindy Ferrer, head of a regional civil defense office, said “[Typhoon Phanfone is] like the younger sibling of Haiyan. It’s less destructive, but it followed a similar path”. Philippine Red Cross chair Richard Gordon, in remarks to the BBC, said “A lot of people have lost their homes and they need food”. He also said many areas lost water and power, and might not get them back for weeks.

Typhoon Phanfone continued westward on Thursday, toward the South China Sea and Asia.

Report urges Kenya to ban plastic bags

A report issued on Feb. 23 by a cadre of environment and economics researchers suggested that Kenya should ban the common plastic bag that one gets at the checkout counter of grocery stores, and place a levy on other plastic bags, all to combat the country’s environmental problems stemming from the bags’ popularity.

Honolulu man dies after fall into sewage-contaminated harbor

A Honolulu man who fell into the sewage-contaminated Ala Wai Yacht Harbor died Thursday night of a massive bacterial infection that caused the loss of one of his legs, septic shock, and ultimately organ failure.

Oliver Johnson, 34, a Honolulu mortgage broker, died between 9:15 and 9:30 p.m. HST Thursday (0715-0730 UTC Friday) when his family removed him from life support. On Friday, March 31, Johnson had apparently fallen into the waters of the Ala Wai Yacht Harbor, located at the mouth of the Ala Wai Canal on the western side of Waikiki.

At the time, the canal and surrounding beaches were contaminated by over 48 million gallons of untreated sewage. The extent to which the sewage spill contributed to Johnson’s symptoms remains unclear.

Johnson earlier reported that he had fallen accidentally into the harbor, then later indicated that he had been involved in a fight on board a boat and had been pushed or thrown into the harbor waters. A security guard at Johnson’s apartment reported that he was bloodied and soaking wet, and that he stumbled into the lobby and collapsed. Paramedics took Johnson to Straub Hospital where he was treated and released.

Johnson reported steadily worsening leg pain that weekend and was admitted to Queen’s Hospital on Sunday with breathing difficulties. Doctors that night amputated his left leg above the knee to try to halt the spread of the infection, and Johnson was placed in a medically induced coma.

Initially suspecting that Johnson had contracted streptococcus-caused necrotizing fasciitis, doctors later found that Johnson had contracted three different bacteria, two of which, vibrio vulnificus and aeromonas hydrophila, have flesh-eating properties. Despite the amputation, Johnson’s condition steadily worsened to the point where doctors said that his other leg and left arm would also have needed to be amputated to save his life.

An autopsy performed by the Honolulu medical examiner’s office reported that Johnson had suffered from massive organ failure caused by septic shock. The medical examiner noted that Johnson had a vulnificus infection on his foot and suffered from chronic alcoholic liver disease which may have contributed to the infection’s taking hold.

The Honolulu Police Department has opened a third-degree assault case; with Johnson’s death, homicide detectives are also investigating. State health officials are also investigating Johnson’s illness.

Johnson’s family has retained an attorney to investigate the causes surrounding his illness and death. It is not clear yet whether any legal action will be taken.
